Volunteer Application Terms & Conditions

All candidates will provide the following references prior to final acceptance: (2) Two written references from any individuals who have known you for a minimum of 2 years and who are able comment on the appropriateness of your strengths and abilities in relation to this volunteer role. Form letters will be provided at the pre-training interview. (1) One reference who will be contacted by telephone. Please provide the name, telephone number and relationship for one individual who has known you for more than 2 years. In addition to the reference checks, all volunteers working with clients will be asked to complete a Police Records Check.

I authorize investigation of all statements herein and release Toronto Commandery Hospice and all other from liability in connection with same. I also understand and verify that the information herein is complete and accurate and that untrue, misleading or omitted information herein may result in dismissal regarding the time of discovery by Hospice. All statements become part of any future volunteer personnel files and will be kept strictly confidential.
